At Conclusive Coordination, we take pride in being a locally owned business that offers NDIS support coordination and recovery coaching services within NDIS plans. However, we are only able to assist plan managed or self-managed clients at the moment, and regrettably cannot extend our services to agency managed clients.
Located in the southern suburbs of Adelaide, we cater to clients situated in the northern, eastern, western, and southern suburbs, as well as the Fleurieu Peninsula. Additionally, we offer remote assistance through virtual platforms like Microsoft Teams or phone consultations.
To ensure our clients receive the best possible support, we actively engage in networking within the local community. This enables us to stay informed about available services in the area, allowing us to make appropriate connections for our clients.
At Conclusive Coordination, our Support Coordinators prioritize acting in your best interest. Our primary objective is to assist in implementing your NDIS plan, providing guidance, and empowering you to make informed choices. Recognizing that there is no "one size fits all approach" in Support Coordination, we acknowledge that each individual has unique circumstances, goals, and limitations. Consequently, we emphasize thinking outside of the box and being creative within the boundaries of our guidelines and roles while offering support.

Connect you to Supports and Services:
Our role is to connect individuals with appropriate mainstream services if deemed necessary. We aim to ensure that clients receive the support they require by facilitating access to relevant services.
Establish and assist in maintain your supports:
Once suitable services have been identified, we assist clients in navigating service agreements, negotiating prices, and ensuring continuous support. Our goal is to help individuals maintain their supports effectively.
Coaching, Refine, and Reflect:
We provide guidance to clients in navigating their NDIS plans, boosting their confidence with the system, and conducting regular check-ins with relevant supports. Our aim is to foster growth and progress within the framework of the NDIS.
Reporting to the NDIA:
We take our responsibility to ensure the care, wellbeing, funding, and safety of our clients seriously. Therefore, any suspicious activity or concerns brought to our attention are promptly reported to the NDIA.
Prepare for an unexpected event:
In the event of unforeseen circumstances such as hospital admissions, changes in living situations, or issues with informal supports, we work closely with our clients to develop a plan that ensures their continued support during such events.
Crisis Situations:
While Support Coordination is not designed as a crisis or on-call response service, we strive to assist individuals in finding appropriate services during business hours when necessary. This may involve connecting clients with crisis services such as justice, education, child and family protection systems, or government homelessness services.
Acting in your Best Interest:
At all times, we recommend and provide supports and services that are tailored to meet your specific needs. We are committed to declaring and avoiding any real or perceived conflicts of interest in order to act in your best interest.
